About Us

CAC Home Health Care is a fully operational Home Health Agency offering skilled nursing, therapy, and personal care services for senior citizens in their own homes.
Skilled services, such as nursing and therapy, are available for home bound Medicare patients that have a skilled need covered under Medicare guidelines. For example, many patients receive skilled services after being discharged from the hospital or rehab facility. Other patients may qualify for skilled care after an injury or while learning to cope with diabetes.
Personal Care Services consist of light housekeeping and assistance with activities of daily living. Seniors may access personal care assistance through a number of ways. Many seniors are eligible of PASSPORT. PASSPORT is a Medicaid Waiver program which provides personal care and homemaker services for Medicaid eligible patients 60 and over who meet the care requirements. Other seniors may be eligible through the Old Americans Act program. Older Americans Act funding is very limited. Personal Care Services are also available for private pay customers at a very reasonable rate.
