Tri-County Overview
There are four Family Health Center locations throughout the
Tri-County area of Jackson, Pike, and Scioto counties, in Ohio. They are located in Jackson,,Waverly,,Piketon, West PortsmouthThe Family Health Centers employ a variety of medical and dental practitioners to better serve patients needs.
  • At the Waverly site there are family practice physicians, a
    pediatrician, a nurse practitioner and OB/GYN services.
  • The new facility in Piketon is home to the Family Health Centers
    dental services.
    This site also has a family physician and a nurse practitioner.
  • The Jackson facility houses one family practice physician, one
    psychiatric physician, one therapist, and a nurse practitioner.
  • The West Portsmouth facility, dedicated primarily to the care
    of children but has recently began accepting family practice
    patients. This facility has a pediatrician, an internist, and a nurse practitioner.
  • Two of the family practice physicians maintain hospital privileges
    at Pike Community Hospital. The two pediatricians on staff and an internist have their hospital privileges at Southern Ohio Medical
    Center in nearby Portsmouth.
  • Family Health Centers accept Medicaid, Medicare, most major insurance
    and provides services based on a sliding fee scale.
  • Family Health Centers provide a full range of services including
    pediatrics, family planning, OB/GYN, and general medicine.
  • Another benefit offered to patients of the Family Health Center is discounted prescription prices through the Kroger Pharmacy.
    Kroger Pharmacy offers convenient locations in Waverly,
    Jackson, and Portsmouth. All Family Health Center patients
    are eligible.
  • The Family Health Centers received national recognition for their
    work in the Diabetes Collaborative and is now participating in
    a national Cardiovascular Collaborative. Family Health Centers
    also offer free diabetic education classes.
  • Medicaid and Managed Care: Each of the four Family Health
    Center sites is a Federally Qualified Health Center (FQHC)
    and accepts all Medicaid Managed Care Plans.
